Acidente com material infectado

O profissional de saúde deverá ser acompanhado pelo período de 6 meses após acidentes com material infectado pelo HIV e em acidentes com paciente-fonte desconhecido. Em exposições com paciente-fonte anti-HIV negativo, o acompanhamento do profissional acidentado somente estará indicado caso haja possibilidade de exposição do paciente-fonte ao HIV nos últimos 3 a 6 meses (possibilidade de "janela imunológica").


Deverá ser realizada avaliação clínica com o objetivo de detectar sinais e sintomas de infeção aguda pelo HIV, que, usualmente, ocorrem de 3 a 4 semanas após a contaminação e incluem febre, adenopatias, faringite e erupção cutânea maculo-papular-eritematosa (Síndrome de mononucleose-símile). Essa sintomatologia está presente em cerca de 80% dos profissionais que soroconvertem.

O acompanhamento sorológico anti-HIV (ELISA) deverá ser realizado no momento do acidente, sendo repetido após 6 e 12 semanas e pelo menos 6 meses. A realização de teste anti-HIV deverá ser feita após aconselhamento pré e pós-teste, devendo ser garantido ao profissional a confidencialidade dos resultados dos exames. A coleta para o teste anti-HIV, no momento do acidente, é importante, para posterior caracterização de infecção pelo HIV em decorrência do acidente profissional. Um profissional de saúde com teste anti-HIV reativo, no momento do acidente, deverá ser esclarecido que este resultado não se deve ao acidente e encaminhado para acompanhamento médico específico.

O profissional deve ser acompanhado por um ano, especialmente, nas seguintes condições: sintomas de possível infecção aguda pelo HIV durante os primeiros 6 meses de acompanhamento após o acidente, uma história clínica prévia sugerindo uma deficiência de resposta imune e a exposição ocupacional simultânea ao vírus da hepatite C. 


O profissional de saúde deverá ser orientado durante o período de acompanhamento para adotar medidas para prevenir a transmissão sexual (utilizando preservativos) e através de sangue, e para evitar a doação de sangue/órgãos, gravidez e aleitamento materno.

Nanosuperficies de Implantes e alterações alveolares




Aims: To describe the early phases of healing at the alveolar ridge around dental implants placed into fresh extraction sockets and to study whether (i) the dimension of the socket and (ii) a new implant surface nano-topography may have any influence. 


Materials and Methods: Sixteen beagle dogs received 64 test (new surface) and control implants randomly placed at the distal socket of 3P3 and 4P4. The implant shoulder was levelled with the marginal buccal bone crest. Animals were sacrificed at 4 h, 1, 2, 4 and 8 weeks for histological examination.

Results: Bone loss occurred at the buccal crest between the 4-h and 1-week healing intervals, being more pronounced at the third premolar site [vertical bone loss between day 0 and 8 weeks 1.1 (0.5) mm]. The corresponding loss at fourth premolar site was 0.3 (0.5) mm. Test sites containing implants with discrete crystalline deposition nano-particles’ surface exhibited less buccal bone resorption than control sites at 8 weeks.


Conclusion: Dimensions of the socket influenced the process of wound healing of implants placed int the o fresh extraction sockets, with more bone loss in the narrower sockets; however, the implant surface nano-topography seemed to have a limited effect in the healing of this implant surgical protocol.

mplant stability in relation to osseointegration: an experimental study in the Labrador dog

Abstract 


Objective: Resonance frequency analysis (RFA) is supposed to determine implant stability. The relation between RFA and the degree of bone-to-implant contact (BIC), however, is unclear.The objective of the present experiment was to evaluate RFA values in relation to osseointegration.


Material and methods: In 20 Labrador dogs, all mandibular premolars were extracted bilaterally. After 3 months, four transmucosal screw-shaped experimental implants were placed in each mandibular premolar region. The implants (12 mm length, Ø 4.1 mm, insertion depth 9 mm) were either SLA surface or turned surface implants. The animals were divided into four groups (five dogs in each group) to study healing following implant installation at 2 h, 4 days, 1, 2, 4, 6, 8 and 12 weeks. Two experimental implants of each type were installed in each edentulous premolar region. A plaque control program was initiated 2 weeks after each implant installation. RFA assessments were performed at the time of implant installation, at one to three occasions during the monitoring period and at the termination of the experiment. At the end of the experiment, the dogs were sacrificed and each implant site was dissected and processed for histological analysis. The results of the histological analysis, i.e. marginal bone level, degree of osseointegration (BIC%) and bone density, were compared with the corresponding Implant Stability Quotient (ISQ) values of the RFA assessment. 


Results: No correlations between histological parameters of osseointegration and ISQ values could be identified. Marginal bone level changes, differences in BIC% and bone density were not reflected in the RFA at any time-point during the 12-week monitoring period. 


Conclusion: The value of RFA to predict implant stability over time and to determine at which time-point an implant may be exposed to functional load has to be questioned.

Early Loading

Consensus statements and recommended clinical procedures regarding loading protocols for endosseous dental implants.

Confusion is still present among clinicians and researchers about what constitutes early loading. This statement defined early loading as a restoration that is in contact with the opposing dentition and placed at least 48 hours after implant placement but not later than 3 months afterward. It is obvious that the time frame is very flexible, and more consensus is necessary to craft terminology that more accurately reflects quickly changing clinical protocols.

Antibiótico e Falhas Implantares

Does antibiotic prophylaxis at implant placement decrease early implant failures? A Cochrane systematic review


Conclusions 
There is evidence from a meta-analysis including four trials with 1007 patients suggesting that 2 g of amoxicillin given orally 1 h preoperatively sig- nificantly reduces early failures of dental implants placed in ordinary conditions. More specifically, giv- ing antibiotics to 33 patients will avoid one patient experiencing early implant losses. No statistically sig- nificant differences in post-operative infections and adverse events were observed. No major adverse events were reported. It might be sensible to suggest a routine use of a single dose of 2 g of prophylac- tic amoxicillin just before placing dental implants. It remains unclear whether an adjunctive use of post- operative antibiotics is beneficial, and which is the most effective antibiotic.
Priority in future research should be given to large pragmatic double-blinded RCTs evaluating the effi- cacy of prolonged antibiotic prophylaxis when com- pared to a single preoperative dose into those sub- groups of patients where implant failures are more likely to occur, particularly in those patients receiving immediate post-extractive implants and augmenta- tion procedures in conjunction with implant place- ment. It would also be useful to investigate the most effective antibiotic type.




Jalecos contaminados

Contaminação atinge 95% dos jalecos médicos

Em vez de proteger o usuário, o jaleco médico - indicado pela Organização Mundial de Saúde (OMS) como equipamento de proteção individual para os profissionais do setor - pode ser fonte de contaminação. É o que indica um estudo realizado por alunas da Pontifícia Universidade Católica (PUC-SP), campus de Sorocaba.
Das amostras analisadas, 95,83% estavam contaminadas.
Entre os micro-organismos identificados nos jalecos está o Staphilococcus aureus, bactéria considerada um dos principais agentes de infecção hospitalar.
A proposta surgiu após a constatação de que alunos e residentes do hospital-escola do Conjunto Hospitalar de Sorocaba, da rede estadual de saúde, saíam para o almoço em bares e restaurantes sem tirar o jaleco.
"Essa elevada taxa de contaminação pode estar relacionada ao contato direto com os pacientes, aliada ao fato de os micro-organismos poderem permanecer entre 10 e 98 dias em tecidos, como algodão e poliéster", explica Fernanda Dias,uma das alunas da PUC-SP.


A PUC-SP pretende aprofundar os estudos para encaminhá-los à OMS.
